Our organization is utilizing the extended workflow capabilities that comes with iLove SharePoint. Recently, we've witnessed that in some instances a workflow is never triggered against an item when it is first created.
After attempts to figure out the issue, we began to discover the following in the ULS logs at the same time the item is created and the Workflow timer job is kicked off:
